CIRCUMCISION CONTROVERSIES.

Circumcision Controversies.

Male circumcision has often been, and remains, the subject of controversy on a number of grounds - including religious, ethical, sexual, and health.

The Ancient Greeks and Romans valued the foreskin and were opposed to circumcision – an opposition inherited by the canon and secular legal systems of the 'Christian West' that lasted at least through to the Middle Ages, according to Hodges.

Traditional Judaism and Islam have advocated male circumcision as a religious obligation.

The ethics of circumcision are sometimes controversial.

From the mid-19th century, there has been advocacy in some Anglophone countries on medical grounds, such as the prevention of masturbation and "reflex neurosis".

Modern proponents, such as Morris, argue that circumcision reduces the risks of a range of infections and diseases as well as conferring sexual benefits.

In contrast, opponents, particularly of infant circumcision, often question its effectiveness in preventing disease, and object to subjecting newborn boys, without their consent, to a procedure they consider to have debatable benefits, significant risks and a potentially negative impact on general health and later sexual enjoyment.

GENITAL MODIFICATION AND MUTILATION.

GENITAL MODIFICATION AND MUTILATION.

The terms genital modification and genital mutilation can refer to permanent or temporary changes to human sex organs.

Some forms of genital alteration are performed at the behest of an adult, with their informed consent. Others are performed on infants or children.

Any of these procedures may be considered modifications or mutilations by different groups of people.

HIV SYMPTOMS.

HIV SYMPTOMS.

Many of the questions we answer on this Forum and also the Freedomhealth Sexual Health Forum relate to anxiety about HIV and also possible early HIV symptoms.

One of the main problems in relying on HIV symtoms themselves is that they are notoriously vague and almost all the symptoms may in fact be symptoms of a different disease.
The best method always of diagnosing HIV is to have a modern HIV test.

Diagnosis may be difficult and uncomfortable but in Western Industrialised countires knowing your HIV status - positive or negative is ALWAYS a good thing.
Knowing you are HIV positive and engaging in medical care will preserve your life to a normal lifespan.

It sounds obvious, but in order to acquire HIV there has to be a significant exposure.    A significant sexual exposure means unprotected vaginal and or anal (or both) penetrative sex with a person who already has HIV infection.

Dr Martin Fisher of Brighton and Sussex University Hospitals in the UK has looked specifically at HIV sero-conversion illness and has found that it occurs much more commonly than is currently thought.

We knew already that early HIV disease provides a short interval at an early stage in the illness to identify new infections. By identifying new HIV infections early we can do two important things.

Firstly we can dramatically alter the health of the person we have just diagnosed.
This is because we are in an informed position where we can prevent that person entering a long period of several years chronic undiagnosed infection which ultimately ends in a crisis catastrophic illness where the diagnosis is made.
This long period of slowly deteriorating health can be halted by just knowing a persons HIV status.
The crisis point where a previously undiagnosed HIV positive patient becomes critically ill will frequently end their life.

Secondly, we can dramatically alter the onward transfer rate of HIV to other people. Dr Fisher, in his contribution to the UK's National Aids Trust Policy Report "Primary HIV Infection", cites some research published by Marks in the Journal of AIDS 2005 where a 50% reduction in risky behaviour was seen in people who were aware of their HIV diagnosis.

There was an even greater reduction in risky behaviour when the HIV positive person was aware that their sexual partner was HIV negative.

The possibilities presented by the second of these points are enormous in terms of reducing spread.
Imagine if we could successfully reduce transfer rates by greater than 50% without doing anything special ?
We can. We just need more people to be aware of their status.

So what are typical HIV Symptoms?

Dr Fishers team looked at 108 people with HIV and found that 70 per cent of them had three classic symptoms - the HIV "Triad". These are:-

Severe sore throat  -  High Temperature  -  A body rash.

These three classic symptoms and signs in an adult should suggest a possible HIV infection and should generate a suggestion that that person should have an HIV test.

There are many other symptoms and signs such as severe muscle aches, headaches, night sweats, lymph node enlargement, and joint pains.

All of these are valid but the key three features are as given above -
1) very high temperature
2) very sore throat
3) body rash.

These three classical symptoms should point to the need for an HIV test, preferably one or a mix of a 4th generation HIV DUO test and/or an HIV PCR test.

Both of these will allow us to make an early diagnosis with the option then of stopping a severe seroconversion illness early and preventing early immune damage and also allowing the newly infected HIV positive patient to limit the accidental further spread of HIV.

COMPLICATIONS OF CIRCUMCISION.

COMPLICATIONS OF CIRCUMCISION.

Circumcision remains a common operation, with over 30,000 procedures performed annually in the U.K., mostly on children.
The British Medical Association has recommended that circumcision should be performed only for medical reasons.
Despite this, controversy exists over whether too many circumcisions are being performed.
Are patients being exposed to an unnecessary operation?

It may be argued that in doubtful cases it is easier to proceed to circumcision on the assumption that the risks are low, but the operation is associated with a definite morbidity and rare deaths are reported.
This review considers the spectrum of complications what may result from circumcision and discusses the possible aetiological (the study of causation) mechanisms.

MEN FEAR CIRCUMCISION.

MEN FEAR CIRCUMCISION.

The male circumcision programme that was launched in 2009 has only seen 10% of the targeted 1,2 million men by 2015 being circumcised, a government official said yesterday.

The programme’s national coordinator in the Health and Child Welfare ministry, Sinokuthemba Xaba, was speaking during the opening of a voluntary medical male circumcision clinic in Bulawayo’s Lobengula suburb.

He said most males feared getting circumcised due to a number of factors.

“More and more men have heard about the benefits of male circumcision, but have various concerns that prevent them from getting circumcised,” Xaba said.

“Males have questions like; what if something goes wrong?

“Will the wound heal? Where will my foreskin go?
These are some of the fears that we hear every day from men who are considering going for the procedure.”

He indicated male circumcision was a simple and safe procedure that took less than 20 minutes and was conducted by highly trained doctors and nurses.

“During the procedure, you do not feel any pain as it is performed under local anaesthesia,” he said.

“With good hygiene and care, the healing process is fast and foreskins are placed in plastic bags (?) and incinerated as per Ministry of Health and Child Welfare guidelines and the Human Tissue Act of Zimbabwe.”

Xaba also said the ministry wanted to expand male circumcision services to all districts nationwide.

He said the provision of more of these services will help to ensure that they were readily available even in remote rural areas.

Xaba said government was still a long way from reaching its target of having 1,2 million men circumcised by 2015.

“We need to further consolidate our partnerships with all stakeholders in the health sector, religious and traditional leaders and the media to help their communities better understand the benefits of male circumcision to men,” he said.

Bulawayo mayor Thaba Moyo, who officially launched the clinic, commended the opening of the facility — which will be the second one in Bulawayo.

“As residents of Bulawayo and Lobengula, you may be wondering why the city of Bulawayo is committed to the voluntary medical male circumcision programme?

“Male circumcision works as the ministry has carefully analysed evidence from the clinical trials conducted in three African countries, which showed that male circumcision offers 60% protection to men against HIV infection,” Moyo said.
(If it only gives 60% protection - what's the point in the first place!!!!)

MASTURBATION.

MASTURBATION.

"A SOLO PLEASURE"

Almost all boys masturbate (let's say 98%).
Those who don't may be extremely sexually repressed, have a low sex drive, or have sexual/emotional problems that might result from childhood emotional or sexual abuse, such as being forced to absorb fundamentalist religious views imposed on them by their family.
Even so, many boys in this situation still masturbate, and try to deal with the guilt afterwards.
In my book, the oppression of healthy, normal male sexual instincts during puberty by religious fanatics ranks as a crime against the soul.
Only the sexually weird and the socially or emotionally disturbed would try and tell a teenage boy that masturbation is abnormal or sinful.
Men are born with a powerful sexual urge, and expressing it is fundamental to normal male development.

So why so much guilt?

The profound pleasures of masturbation, which releases sexual tension and enables a boy to learn about his body, is something to be honoured.
Guilt comes form the repressed guilt and shame of those who bring a boy up: it's not inherent to the act of masturbation!
I'd say that a boy who doesn't masturbate has a problem - not one who does. (Unless it becomes obsessive or happens many times a day.)
Masturbation needs to be celebrated, not allowed to induce guilt.
But overcoming the guilt can be difficult - for many men, it was instilled at the hands of their mother, at a very early age, when she told them not to touch themselves "down there".
Well, there never was going to be a less effective instruction to a boy than that!

But of course the consequence of our mothers' difficulty with our innocent sexuality when we were babies may be that the experience and enjoyment of our own bodies is taken away from us, or we are made to feel ashamed in some way of our maleness and what we can do with it: hardly a good position from which to achieve true emotional health.
The question is, what can men do to feel better about masturbation?
One thing's for sure, men will do it, whether or not they feel guilty or bad about it.
The answer, then, lies in changing our attitudes so that we can enjoy masturbating, and indeed celebrate it, as an essential part of our maleness.
Perhaps our masturbation becomes more controlled as we get older but it still goes on, even when men have sexual partners.
For heterosexual men, masturbation is often secret, and a wife who discovers what her man is doing will fail to understand his need to masturbate, and assume in some way it reflects badly on her.

We need to explain that it is a normal part of our experience of maleness, a healthy and rewarding thing to do.

"Male circumcision should be banned"

All forms of female circumcision were banned in the UK in 2004.

Male circumcision should be legislated against in a similar manner.
